If you haven’t already done so, please make plans today to get your VBWellnessforLife screening (for 2014). These screenings are required each year if you want to be eligible for the $500 health care premium credit.
- Once you get your screening, you must submit your form by Oct.31, 2013 to be eligible to receive your $500 health care premium credit for 2014.
- You may complete your health screening through one of the following options:
- Wellness & Prevention Screening (Offered at various City and school sites throughout the year. See remaining schedule below.)
- Annual Physical with Primary Care Physician or Occupational Health (Occupational Health is for City public safety and safety sensitive active employees only); if you use your primary care physician you are responsible for sending the completed form to Optima Health.
- Sentara Reference Lab (9 Designated Labs– see back of lab form for locations)
- If you can’t remember if you’ve been screened for next year, you can check the status of your health screening by calling the VBWellnessforLife hotline at 552-7287; leave a message with your name, date of birth and phone number and someone will call you back to let you know.

Wellness & Prevention does not recommend allowing your physician to submit your health screening form. (That’s like leaving $500 in someone else’s hands!) In the future make sure you are the one submitting your form to Optima Health and keep a confirmation of submission by sending it via certified mail or keeping a copy of your fax confirmation.
If you would like to verify your form was submitted, you may call the health screening hotline at 757-552-7287 and leave a message with your name, date of birth, and phone number you can be reached at and someone from Optima Health will return your call.

Wellness & Prevention screenings are all walk-in clinics. No registration required.
