Health care credit information

You have the opportunity to earn an annual $500 health care premium credit to be applied to your health care costs. All you have to do is complete a health screening by Oct. 31, 2012.

The screenings are free, and include testing for cholesterol, blood pressure, diabetes, a BMI analysis as well as questions regarding smoking and physical activity.

You can undergo these screenings at your annual physical with your primary care physician. You can also get the screening at any one of the nine designated Sentara Reference Labs (see list below), or at a Wellness and Prevention Screening, which are available at various city and school locations through October.

In December, all screened participants will receive a letter. If, during the screening, you are deemed to have two or more health risks, you will be required to participate in quarterly health coaching through our partner, Healthyroads™. This includes telephonic coaching sessions on weight management, tobacco cessation, stress management and healthy living.

Additionally, all screened participants – regardless of the number of risk factors – have access to award-winning program materials, such as manuals and self-care guides; and, access to e-tools including nutrition and fitness planners, animated exercises, e-coaching courses, “Ask a Coach” section, daily email tips on personalized topics, health articles and a personal health calendar.

Make sure to download the health screening form here, have it filled out during your screening and send the completed form to Optima Health, Attn: VBWellnessforLife, 4417 Corporation Lane, Virginia Beach, VA 23462. Or, fax it to Optima Health at 757-687-6159.

All full time and full time equivalent school employees on a VBCPS Optima Health insurance plan or retirees on a VBCPS Optima health insurance plan are eligible for this program.
